Exalta Health reflects the spirit of Christ by providing compassionate, accessible healthcare. We envision a collaborative community where accessible quality healthcare promotes wellness for all. We provide primary medical and dental care along with spiritual care and counseling on a sliding fee scale to the un-insured and under insured who are at or under 250% of the Federal Poverty Index.
Exalta Health is a non-profit, faith-based health center in the Burton Heights area of Grand Rapids. The founders and supporters of Exalta Health, and those who volunteer their time, do so because of their love of Jesus and their desire that you experience health in body, mind and spirit.

All of our health care providers are licensed professionals and are committed to increasing your individual well-being as well as the overall health of our community.
Medical Primary Care that includes:
- Health assessment
- Treatment of chronic illness
- Urgent Care
- Specialty care in the clinic
- Referrals for off site appointments
- Assistance for low cost medications
- Reduced cost pharmacy medications
- Eye exams and glasses. You will be responsible to pay for lenses.
- Dental Care
- Dental exams
- Treatment and/or referrals
- Dental hygiene
- Oral health education
Education on a variety of health topics are available to you. Diabetes patients can be referred to other programs or remain at Exalta Health for a series of classes for diabetes education. We encourage all patients to invest time and effort in their medical care by asking for further explanation on their medical situation. It is valuable for you to keep track of your visits/procedures and medications.
Counseling services are available to our medical patients. Your provider may recommend this service to you or if you are interested in counseling services please talk with your provider.
You may talk with a someone to find what other social service benefits you may qualify for from other organizations.
Spanish Interpreters are available during clinic hours.
Exalta Health offers prayer, hospital and home visits, and worship center referrals. There is a Chaplain available to speak with you concerning any issue you have but particularly about issues related to faith and spirituality. Please feel free to ask a chaplain to pray with you or just simply to chat.
Exalta Health is able to loan or give you walkers, wheelchairs, and other rehabilitation equipment that we have available
Exalta Health patients are uninsured or enrolled in Priority Health Medicaid, Meridian Medicaid, or West Michigan Wellnet. We do not take patients with regular insurance. We ask that you apply for Medicaid or an insurance through the marketplace, please make an appointment and we will assist you with that process. If you are seeking any other health related service please ask one of the staff members and we will be happy to explain what can be done on your behalf.
